Baptism Form

Matthew 28:18–20

“Then Jesus came to them and said, ‘All authority in heaven and on earth has been given to me. Therefore go and make disciples of all nations, baptizing them in the name of the Father and of the Son and of the Holy Spirit, and teaching them to obey everything I have commanded you. And surely I am with you always, to the very end of the age.’”

Baptism is a beautiful outward expression of an inward faith in Jesus Christ. It is a public declaration that you belong to Him, have been made new through His grace, and desire to follow Him in obedience.

At Jesus House Ocala, we believe baptism is an important step for every believer. It does not save us, but it points to the saving work of Jesus — His death, burial, and resurrection — and shows that our old life has been buried with Christ and that we have been raised to walk in new life with Him.

Please complete this form if you are interested in being baptized. One of our Elder Shepherds will reach out to you with more information and help walk you through the next steps. Thank you!
